At the same time, deficits in communication between systems sharing patients can not only lead to confusion and waste, but also to increased risk of harm. Case Management and Care Coordination offers an evidence-based framework, best practices, and clinical common sense to meet this ongoing challenge. Focusing on families of children with chronic health issues, it outlines the processes of case management and care coordination, clarifies the roles and responsibilities of team members, and models streamlined, patient-centered service delivery. This analysis cuts through much of the complexity of case management while emphasizing collaboration, flexibility, and advocacy in pursuing best outcomes for patients. And as an extra dimension of usefulness, the book is accessible to lay readers, empowering families to make informed decisions and have a more active role in their own care. Included in the coverage:   Essential skills for integrated case management. Children and youth with special health care needs. Transitional care and case management settings for children and families. Case management and home visitation programs. Managed care and care coordination. Technology and care coordination. Effectively illustrating the possibilities and potential o f health care reform, Case Management and Care Coordination is an essential resource for pediatricians and health care professionals, as well as for families of children with special health care needs.SpringerBriefs in Child Health,2625-2872 ;0PediatricsMaternal and child health servicesSocial servicePediatricshttps://scigraph.springernature.com/ontologies/product-market-codes/H49006Maternal and Child Healthhttps://scigraph.springernature.com/ontologies/product-market-codes/H27025Social Workhttps://scigraph.springernature.com/ontologies/product-market-codes/X21000Pediatrics.Maternal and child health services.Social service.Pediatrics.Maternal and Child Health.Social Work.362.19892Treadwell Janetauthttp://id.loc.gov/vocabulary/relators/aut868570Perez Rebeccaauthttp://id.loc.gov/vocabulary/relators/autStubbs Debbieauthttp://id.loc.gov/vocabulary/relators/autMcAllister Jeanne Wauthttp://id.loc.gov/vocabulary/relators/autStern Susanauthttp://id.loc.gov/vocabulary/relators/autBuzi Ruthauthttp://id.loc.gov/vocabulary/relators/autQCQUBOOK9910304132403321Case Management and Care Coordination1938820UNINA03539nam 22006135 450 991015054250332120200630105300.0978981100804710.1007/978-981-10-0804-7(CKB)3710000000938002(DE-He213)978-981-10-0804-7(MiAaPQ)EBC4738840(PPN)197138527(EXLCZ)99371000000093800220161110d2017 u| 0engurnn|008mamaatxtrdacontentcrdamediacrrdacarrierChiral Four-Dimensional Heterotic String Vacua from Covariant Lattices /by Florian Beye1st ed. 2017.Singapore :Springer Singapore :Imprint: Springer,2017.1 online resource (XII, 95 p. 3 illus.) The author provides a systematic classification of potentially realistic heterotic covariant lattice vacua, which possess a lower number of moduli fields when compared to conventional compactification methods, by means of number theoretical methods. These methods, while well known to the mathematics community, have not yet found many applications to physics. They are introduced to the degree necessary to understand the computations carried out throughout this work. Furthermore, explicit covariant lattice models with particularly interesting properties are analyzed in detail. Finally, new light is shed on the relation between covariant lattice models and asymmetric orbifold compactifications, the result being a concrete correspondence between certain types of asymmetric orbifolds and those classified covariant lattices.Springer Theses, Recognizing Outstanding Ph.D. Research,2190-5053Quantum field theoryString modelsParticles (Nuclear physics)Mathematical physicsQuantum Field Theories, String Theoryhttps://scigraph.springernature.com/ontologies/product-market-codes/P19048Elementary Particles, Quantum Field Theoryhttps://scigraph.springernature.com/ontologies/product-market-codes/P23029Mathematical Applications in the Physical Scienceshttps://scigraph.springernature.com/ontologies/product-market-codes/M13120Quantum field theory.String models.Particles (Nuclear physics)Mathematical physics.Quantum Field Theories, String Theory.Elementary Particles, Quantum Field Theory.Mathematical Applications in the Physical Sciences.530.15Beye Florianauthttp://id.loc.gov/vocabulary/relators/aut825102MiAaPQMiAaPQMiAaPQBOOK9910150542503321Chiral Four-Dimensional Heterotic String Vacua from Covariant Lattices1835295UNINA01315nam0 22002651i 450 UON0018092320231205103131.56722-7500-925-620030730d1986 |0itac50 bafreFR|||| 1||||ˆLes ‰associations d'entreprises (joint ventures) dans le commerce international Liuz O.
