A reissue of the 1637 edition (STC 4788) with "The epistle to the reader" ((a)-(b)⁴) added and the two final digits of the date erased from t.p. and "41" substituted in MS. Variant 1: with the epistle, and date changed to 1645. Variant 2: with the epistle, and date changed to 1647.

The original edition was possibly printed in Leiden by W. Christiaens; the epistle was probably printed in London (STC).

Identified as STC 4788a on UMI microfilm "Early English books, 1475-1640" reel 1166. Identified as Wing P3922 on UMI microfilm "Early English books, 1641-1700" reel 222.
